With this document, you authorize a person you trust to represent you at the Workers’ Compensation Board in your proceedings. This person must be a member of your family. Family members may act on your behalf, but friends are not allowed to do so. You still want to receive the mail from the Employer’s Liability Insurance Association yourself.

It is important that you send the power of attorney to the Employer’s Liability Insurance Association. This will allow the authorized person to officially represent you and you will continue to receive your official mail.
